BEFORE YOUR APPOINTMENT
Every signer must appear in person. Please bring your original identification and discuss any ID concerns with Herica before booking.
California law generally requires identification to be current or issued within the past five years. Common options include:
A California prison inmate ID or sheriff-issued inmate ID may also qualify when the signer is in the applicable custody.
The following must meet the applicable validity requirements and contain a photograph, physical description, signature, and identifying number:
Some military IDs lack the required information. Please confirm your specific ID with Herica in advance.
This option is available only when the legal requirements are met. It is not a substitute for simply forgetting your ID. Arrange it with Herica before your visit.
Both witnesses must appear in person with you, bring acceptable identification for the notary to verify, and sign the notary’s journal. They do not need to know the notary personally.
Each witness must swear or affirm under penalty of perjury that:
Credible witnesses establish your identity; they are different from witnesses required to sign a legal document. Herica must confirm that all identification and witness requirements are satisfied before notarization.
Based on the California Secretary of State’s Notary Public Handbook (PDF), Identification section, and California Civil Code §1185. Contact Herica before the appointment if your ID is expired, unusual, or unavailable.
FORMS & NOTARIZATION
Common California forms and what to expect at your appointment. These resources are for general information.
I am not an Attorney and cannot provide legal advice, prepare legal documents, or select the notarial certificate for you. If you are unsure whether an Acknowledgment, Jurat, or another certificate needs to be attached, please consult your Attorney or the person or organization that provided your documents before your appointment.
You appear in person, your identity is verified, and you acknowledge signing the document. You may have signed it beforehand; an oath is not required. The notary completes the certificate and journal entry.
Acknowledgment form (PDF)California Secretary of State
You appear in person, your identity is verified, and you sign in the notary’s presence. You also take an oath or affirmation that the document’s statements are true. An affirmation is a nonreligious alternative to an oath.
Jurat form (PDF)California Secretary of State
This legal document authorizes another person to act for you in specified financial or property matters. It is a document to be notarized, rather than a notarial certificate. The linked statutory form includes an Acknowledgment. For notarization, appear in person with acceptable identification; a journal signature and thumbprint are required. Ask your Attorney about the powers and instructions before completing it.
Statutory Power of Attorney (PDF)Sacramento County Public Law Library · Financial matters, not health care
This form includes a Power of Attorney for Health Care and treatment instructions. It provides options for two qualified witnesses or notarization. If you choose notarization, appear with acceptable identification. Skilled nursing facility residents also need the special patient-advocate or ombudsman witness described in the form, even if it is notarized.
